PREPARE FOR YOUR VISIT
On your first visit to our office, our receptionist will ask you to complete a few patient information forms. Or for you convenience, you may fill these forms prior and bring the completed forms to your appointment. If you choose to complete these forms from our website, you may print the forms and complete by hand or fill them out on-line and print once completed. The form on-line will not be saved, so please remember to print once completed.
Please note the questions we ask will provide our staff with essential data on your health and medical history. You will also be asked questions concerning your contact information, insurance and billing information. There may be times when we will need to contact you, so please provide as complete information as possible, including a cell number and email address. Periodically, you will be asked to update your patient file in order to help us provide the best care and service.

APPOINTMENTS
Office visits are scheduled by appointment by calling 386-446-6540 . At times, unfortunately, emergencies and delays are possible. Should we ask you to reschedule your appointment, your patience and understanding is greatly appreciated. We make a sincere attempt to adhere to the office schedule as much as possible. Should something arise where you are unable to make your scheduled appointment, please notify us as soon as possible, so that we may reschedule your appointment for a more convenient time.
REFERRAL NUMBERS
If your insurance company requires a referral for you to see a specialist, please work with your primary care doctor to secure the referral number before you arrive at our office. Following this procedure will help you avoid delays in waiting for this number once you arrive at our office.
